- बिहार राज्य का मूल निवास प्रमाण पत्र,
- सभी विद्यार्थियो का जाति प्रमाण पत्र,
- परिवार का वार्षिक आय प्रमाण पत्र,
- 10वीं कक्षा का अंक पत्र व प्रमाण पत्र ( यदि उपलब्ध हो तो ),
- पासपोर्ट साइज फोटो और
- चालू मोबाइल नबंर आदि।
